To guarantee patient safety, the Pharmacy and Poisons Board requires prescriptions to feature:

Doctor / Clinician full name, signature & registration number

Hospital, clinic, or practice official stamp / letterhead

Patient full name, age/DOB, and date of consultation

Prescription issue date within the last 6 months
